Is it possible to approve a refund at no cost if passengers were required to purchase a new ticket at the airport?

Passengers are required to present proof of the new ticket and provide an explanation for not utilizing the original ticket. Approval of the refund depends on the policy of the operating carrier. If the problem was not caused by the airline's action, a free refund is unlikely unless the fare rules allow it.  

Kindly submit a refund application accompanied by all relevant supporting documents. Refunds will be approved only if they follow the terms and conditions of the operating carrier.
